01What it does

What Curative Connections Inc does

Mission

Curative connections' mission is to help people achieve their goals for independence. We serve families, young adults, seniors and people with disabilities with concerns related to aging, dementia, mental health, brain injury or other disability, or barriers to employment. We help people maintain or regain independence, build skills and provide enriching opportunities for healthy aging.

As stated on its FY2025 Form 990.

Programs

On its FY2025 return the organization reported a significant change in how it conducts a program.

Lack of federal funding for the senior community service employment program (scsep) caused our senior training program to be on hold as of 7/1/2025. That funding took many months to be restored. Before that funding was restored, we made the difficult decision to no longer administer that program, due to the very volatile nature of that funding.

Its own explanation, from Schedule O of that return (Part III, line 3).

  1. Curative connections provides adult day programs with comprehensive support for individuals with dementia, brain injury, stroke, or other cognitive conditions. Additionally, curative connections offers work, life, and social skills development along with specialized transportation services for people with disabilities and our aging population.adult day and dementia care groups offer day program options for older adults and people with disabilities that benefit from a supportive social setting to engage with others, find meaning and purpose in their days, and continue to be a part of their community - with the goal of helping people maintain their community living situation through supportive services. In 2025 we served 279 people; hours of operations: monday - friday 7am-5:30pm. All programs are certified through the state of wisconsin. 94% of those enrolled maintained their living situation in 2025.

  2. Transportation services provided 45,164 distinct trips through brown county for people with disabilities and vulnerable older adults, traveling 523,093 miles. The program uses 47 volunteers and 29 vehicles to transport people to and from a variety of appointments in the community to help them remain connected, safe and healthy. 2025 - served 895 people.

  3. Cognitive rehab and wellness programs support individuals with neurological impairments from a traumatic or non-traumatic brain injury, as well as those who may have a diagnosis of autism, down syndrome, or cerebral palsy. Length of members stay depends on the goals that are set as well as skills and compensatory strategies attained. This program area is split into classroom teaching and the activity-based program. Cognitive rehab and wellness differs from other program areas as programming is not all day every day, a member may only attend one hour of programming to work on a specific skill or competency. Programming and respite can be supplemented with other curative programs, depending on need and compatibility. In 2025 76 individuals attended classes. This service is available monday - friday 9am-3pm.

03Who funds it

Who funds it, year by year

2017 → 2023: the base broadened from 7 funders to 33 funders, grant income rose $170k → $2.9M.

8 of 53 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 21% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.

From the IRS filings of CURATIVE CONNECTIONS INC’s funders (the co-funder graph). Top 30 of 53 funders by total. Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How concentrated its funding is

CURATIVE CONNECTIONS INC has a broad base — no single funder exceeds 15% of grant income, and it takes 5 funders to reach half.

the vertical line marks half of all grant income — 5 funders to its left

42% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund CURATIVE CONNECTIONS INC.

“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Funder states come from each funder’s own filing. $1.7M arriving through sponsors registered in 7 states is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been.
