What We Care Jacksonville Inc does
Mission
Wecarejax coordinates access to compassionate care for uninsured and under-resourced adult duval county neighbors during their individual healthcare journey.
As stated on its FY2025 Form 990.
Programs
Patient services: designed to provide access to advanced diagnostic and specialty health care for duval county's uninsured adults living at 300% or below the federal poverty level, more than 500 volunteer physicians and providers offer consultation and medical care at no cost to the patient or their family. Patients are referred by a network of safety net primary care clinics to wecarejax. Where donated services are not available, the team raises funds to offset out of pocket expenses for lab tests, medication, supplies, and durable medical equipment. The value of donated care totaled 20,011,652 for the year ended september 30, 2025, serving 2,120 patients with 8,287 services, up 16% over the previous year.

Objectives: reduce dependence on emergency room care by establishing a primary care medical home; mitigating the significant burden of chronic disease that can mean lost yers of productive life; and supporting self-sufficiency. Patients in need of follow-up wound care are also offered both in-clinic and in-home support. The program relies on a certified community health worker, volunteer wound care nurses, a staff rn for clinical support, and funds to support transportation, home medical supplies, and more.

The number of patients with a new diagnosis of cancer in 2025 was 50 and 107 patients had at least one visit with a medical oncologist.

During the year ended 9/30/25, the team processed 4,649 referrals (up 101.6%). Cases remain open for up to a year to address the social drivers of health that often prevent retention in primary care. Jaxcareconnect evaluation data during the pilot indicated that 76% of patients served do not return to an emergency room for non-emergent care within one year.

The organization over time
Each line starts at 100 in 2017, so what you read is the shape rather than the size: 150 means half as much again as 2017, 50 means half. The number beside each label in the key is where it ended. The shaded band is where the middle 50% of its peers' revenue would sit, given their growth.
Balance sheet
Total assets and total liabilities at each year end, in dollars. The largest one-year move in total assets was 2024 to 2025, from $396k to $786k (+98%).
How it's funded, over time
Each bar is one year's revenue split into where it came from, and every bar is the same height — these are shares, not amounts, so a year that raised twice as much looks the same size. Hover a bar for the split.
Government-grant reliance: 2018 16% · 2019 21%. Grants only. Government contracts and fees sit inside program revenue.
96% of WE CARE JACKSONVILLE INC’s revenue is contributions — more reliant on donations than three-quarters of its peers (51% for the typical peer).

Who funds it, year by year
2017 → 2023: the base broadened from 6 funders to 9 funders, grant income rose $348k → $594k.
4 of 24 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 25% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.
From the IRS filings of WE CARE JACKSONVILLE INC’s funders (the co-funder graph). Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
How concentrated its funding is
WE CARE JACKSONVILLE INC has a broad base — no single funder exceeds 22% of grant income, and it takes 4 funders to reach half.
the vertical line marks half of all grant income — 4 funders to its left
85% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund WE CARE JACKSONVILLE INC.
Largest funder’s share by year: 2017 40% · 2018 57% · 2019 86% · 2020 37% · 2021 38% · 2022 24% · 2023 44% — growing more concentrated.
“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
How long its funders stay
42% of WE CARE JACKSONVILLE INC's funders are still giving 3 years after their first grant; 63% give in more than one year at all.
Share of funders still giving k years after their first recorded grant, pooled across every acquisition cohort. A funder counts as retained in a year only if it made a grant that year. Measured to FY2023, the last fiscal year that has finished arriving — a funder cannot be counted as lapsed in a year most filers have not reached.
Where its funders are
81% of WE CARE JACKSONVILLE INC's grant income comes from Florida funders.
In-state vs out-of-state, by year
Funder states come from each funder’s own filing. $1.0M arriving through sponsors registered in 4 states is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
