Recognized as a 5 star patient-centered primary care home by the oregon health authority, our integrated primary care model strives to provide an outstanding patient experience and exceptional clinical results. Our medical, dental, and behavioral health care teams comprised of 186.7 fte (full time equivalents), 42.14 of which are licensed independent providers.
