01What it does

What The Mary Wade Home Incorporated does

Mission

The not-for-profit mary wade home has a mission to provide the highest-quality compassionate care, senior living, and services that promote wellness for older adults in greater new haven. This is achieved through a unique senior care continuum offering both residential and community-based services that meet the needs of older adults, families and caregivers. The continuum includes long-term skilled nursing, short-term rehabilitation, memory care, and an adult day center. The continuum of care allows support to increase or decrease, without uprooting a person from their community. Last year, mary wade served more than 700 individuals and their families.

As stated on its FY2025 Form 990.

Programs

  1. Mary wade's 94-bed skilled nursing facility offers comprehensive, 24/7 medical management, wound care, and pain management for individuals requiring long-term nursing assistance, as well as short-term rehabilitation for patients recovering from joint replacement surgeries, surgery, heart attack, or stroke. These services bridge the critical gap between hospital discharge and returning home, by providing 24/7 medical monitoring and intensive therapies that are often too complex or demanding to manage independently, drastically lowering hospital readmission rates while optimizing recovery for seniors in greater new haven.

  2. Mary wade's historic 45-bed residential care home provides low-income elderly individuals with safe, affordable housing, medical oversight, and vital social engagement. These strategies serve to improve our residents' quality of life, while reducing emergency visits and hospitalizations. Access to daily living support (bathing, dressing and medication management) often prevents minor issues from escalating to major crises. The program's built-in opportunities to socialize help to reduce or prevent isolation, loneliness, and cognitive decline.

  3. Mary wade's medical model adult day center offers structured medical oversight, rehabilitation, socialization, personal care, nutritious meals and snacks, and daily transportation to and from the center to alleviate the physical burden of daily commutes, especially for working families who are caring for aging relatives at home. The program also provides specialized programs and staffing to care for older adults with medical conditions and those who are experiencing alzheimer's disease and dementia.

03Who funds it

Who funds it, year by year

2017 → 2023: the base broadened from 2 funders to 5 funders, grant income rose $96k → $180k.

3 of 13 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 7% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.

Left out of every figure on this page: $3.5M from one payer (the payer shares this organization's board, largest Mw Healthcare Inc). These are real filings, and they are not money THE MARY WADE HOME INCORPORATED raised.

From the IRS filings of THE MARY WADE HOME INCORPORATED’s funders (the co-funder graph). Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How concentrated its funding is

THE MARY WADE HOME INCORPORATED leans on a few funders — its largest provides 77% of grant income and the top three 92%; half comes from just 1 funder.

the vertical line marks half of all grant income — 1 funder to its left

84% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund THE MARY WADE HOME INCORPORATED.

“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How long its funders stay

40% of THE MARY WADE HOME INCORPORATED's funders are still giving 3 years after their first grant; 54% give in more than one year at all.

Share of funders still giving k years after their first recorded grant, pooled across every acquisition cohort. A funder counts as retained in a year only if it made a grant that year. Measured to FY2023, the last fiscal year that has finished arriving — a funder cannot be counted as lapsed in a year most filers have not reached.

Funder states come from each funder’s own filing. $76k arriving through sponsors registered in 3 states is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
