What St Augustine Manor does

Mission

St. Augustine Health Ministries continues the healing ministry of Christ by promoting the quality of life and independence of each person entrusted to our care through a continuum of health care and related services to the chronically ill and aging of our community with special concern for the poor. We promote the sacredness of life in a community that celebrates purpose, individuality, companionship and well-being.

As stated on its FY2024 Form 990.

Programs

  1. Skilled Nursing Home Care: 220 bed skilled nursing facility with an in-house Pharmacy and on-site dialysis. Because of our stellar survey history, St. Augustine Manor was chosen by CMS for a new Risk-Based Survey (RBS) process for which only 10% of eligible facilities get chosen. Federal surveyors oversee Ohio Department of Health surveyors in this process. St. Augustine Manor participated in this experimental RBS process in May 2024 and fared well receiving only 3 low level, low in scope and severity, citations. Since 2023, any/all complaint surveys conducted were unsubstantiated. All patient floors were remodeled/renovated in 2015, including our 67 bed skilled care unit. A Respiratory Wing was added in 2018 and renovations to the rest of the building continue into 2025. We consistently are a 4 star facility according to CMS ratings. We placed 41st on the Plain Dealer's top 50 work places for mid-sized companies, and were the only Skilled Nursing Facility to make the list. We specialize in Pulmonary Care, Rehabilitation, Cardio Vascular Diseases, Infectious Diseases, Wound Care, HIV Care, Respiratory and Clinically-Complex Nursing Care. Medicare-Certified Hospice: Community hospice services extending beyond Cuyahoga County into Lorain, Medina, Summit, Geauga and Lake Counties, as well as a recently remodeled 30 bed inpatient facility with mostly private rooms. With an average daily census of over 93 patients, Holy Family Home/Hospice served over 575 patients in 2024 ranging in age from 34 to 107 and provided over $1 million of free charity care. Holy Family Home has expanded its services to provide palliative care to the community through Holy Family Home Health Care. Holy Family Hospice systematically scores well on patient and family satisfaction surveys, as 96.1% of families who have used our services would recommend Holy Family Hospice to their friends and relatives.

  3. Assisted Living Facility: St. Augustine Towers is a licensed residential care facility with 100 apartments, daily Mass in our chapel on site, and a capacity to serve 110 residents. The Towers focuses on their mission to serve the underserved and downtrodden populations in our communities. The Towers was well below average for the number of citations during a survey conducted by the Ohio Department of Health. St. Augustine Towers consistently scores well on patient and family satisfaction surveys, as 97% of residents who have used our services would recommend St. Augustine Towers to their friends and relatives. The Towers had a perfect survey in November 2024 conducted by Western Reserve Area on Aging. The Towers has been a part of the Medicaid Assisted Living waiver program through WRAAA since 2007. We served an average of 66 residents a month under this program which allows a qualified AL waiver recipient to receive rent subsidies affording them an apartment and all offered amenities. The Towers also serves persons with a VA benefit earned during armed conflict for which the veteran or the spouse receives a monthly stipend that is used for rent. The Towers is contracted with the…

03Who funds it

Who funds it, year by year

2017 → 2023: the base broadened from 1 funder to 4 funders, grant income rose $6k → $266k.

4 of 8 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 54% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.

Left out of every figure on this page: $460k from one payer (the payer shares this organization's board, largest Catholic Charities Corporation). These are real filings, and they are not money ST AUGUSTINE MANOR raised.

From the IRS filings of ST AUGUSTINE MANOR’s funders (the co-funder graph). Top 7 of 8 funders by total. Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How concentrated its funding is

ST AUGUSTINE MANOR leans on a few funders — its largest provides 44% of grant income and the top three 91%; half comes from just 2 funders.

the vertical line marks half of all grant income — 2 funders to its left

100% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund ST AUGUSTINE MANOR.

“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

Where its funders are

54% of ST AUGUSTINE MANOR's grant income arrives through donor-advised or pass-through sponsors, whose addresses record where the money is held rather than where the donor is. Of the $314k that is directly attributable, 96% of it comes from Ohio funders.

Funder states come from each funder’s own filing. $374k arriving through sponsors registered in 4 states is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been.
