01What it does

What Island Rehabilitation and Nursing Center does

Mission

To provide the highest quality of care to all resident patients in a manner that ensures respect, dignity, compassion, sensitivity and concern.

As stated on its FY2024 Form 990.

Program

On its FY2024 return the organization reported a significant change in how it conducts a program.

On july 23, 2020, a membership reorganization agreement was entered into by and between island nursing center, inc. ("irnc"), inrc's sole corporate member, mather health syste, inc. ("mather") and gurwin healthcare system, inc. ("gurwin"). The purpose of the membership reorganization agreement was to transition control of Island Rehabilitation and Nursing Center from mather to gurwin so that it could continue to operate as a not-for-profit skilled nursing facility. Gurwin simultaneously entered into an administrative services agreement with irnc whereby gurwin became the exclusive provider of administrative services to irnc. These services included oversight over the following departments: finance, administration, admissions, marketing as well as all clinical areas. On january 14, 2021, pursuant to the membership reorganization agreement, a closing certificate was executed between gurwin healthcare system, inc. And mather health system, inc., making gurwin healthcare system, inc. The sole member of Island Rehabilitation and Nursing Center, inc. Gurwin continues to provide oversight of day to day operations of Island Rehabilitation and Nursing Center.

Its own explanation, from Schedule O of that return (Part III, line 3).

  1. Island nursing and rehab center provided 36,520 days of inpatient rehabilitation services to patients and residents during 2024.

1 of 2 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 2% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.

From the IRS filings of ISLAND REHABILITATION AND NURSING CENTER’s funders (the co-funder graph). Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How concentrated its funding is

ISLAND REHABILITATION AND NURSING CENTER leans on a few funders — its largest provides 98% of grant income and the top three 100%; half comes from just 1 funder.

the vertical line marks half of all grant income — 1 funder to its left

100% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund ISLAND REHABILITATION AND NURSING CENTER.

Where its funders are

100% of ISLAND REHABILITATION AND NURSING CENTER's grant income comes from New York funders.

NY

Funder states come from each funder’s own filing. $207 arriving through sponsors registered in 1 state is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
