01What it does

What Alamo Area Community Network does

Mission

The Alamo Area Community Network (aacn) is an alliance of community based organizations (cbo's) working to improve the health and well-being of individuals and families in the san antonio region. The aacn brings together human service and nonprofit organizations funders, government entities, healthcare and health plans on a cloud-based, hipaa compliant online platform to coordinate social services across our community.

As stated on its FY2024 Form 990.

Program

  1. Our primary program is the social services platform we offer to non-profits and community-based organizations. More than 15,000 referrals have been sent between our community partners through our client management and social service referral platform. We've also identified the critical needs of the 18,000 individuals served through our tool. This information can be shared with local funders to help inform service gaps.

the vertical line marks half of all grant income — 1 funder to its left

100% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund ALAMO AREA COMMUNITY NETWORK.
