To this end, we seek to establish roots within PR impoverished communities becoming a catalyst of healthy empowerment by fostering inclusiveness, social equity, and resilience. With a strong community presence, our Foundation has experience working alongside community leaders while designing programs focused on identifying populations with complex chronic health conditions and high psycho social needs. Owing to our community-based approach, we have been drivers of multi agency concerted efforts aimed towards the eradication of social and physical determinants of health.
