Community Health Partnership (Chp) was founded in 1992 as a collaborative effort focused on improving coordination between local health care providers. Over the decades, the organization's work has expanded in response to community needs. Acting as a conduit and convener, chp brings together diverse individuals and organizations -including nonprofits, businesses, and government agencies - and equips them with the strategy, support, and skills needed to drive lasting change.
