What Consumer Directed Choices Inc does
Mission
As the premier provider of consumer-directed personal assistance we empower independence and advocate for people with disabilities, seniors, and their families.
As stated on its FY2025 Form 990.
Program
On its FY2025 return the organization reported a significant change in how it conducts a program.
Consumer directed choices (cdchoices) served as a fiscal intermediary for employers (consumers) and employees (personal assistants) under the new york state consumer directed personal assistance program (cdpa) through july 2025. In accordance with state law, the cdpa program transitioned in 2025 from multiple fiscal intermediaries to a single statewide fiscal intermediary. As a result of this transition, cdchoices ceased operations within the cdpa program, which had a significant impact on the organization's revenue.
Its own explanation, from Schedule O of that return (Part III, line 3).
In fulfillment of its responsibilities under various consumer directed programs, cdchoices serves as a fiscal intermediary for consumers/designated representatives. Among its fiscal intermediary services is to provide payroll and benefits services for 3,050 employees of consumers/designated representatives. As such, of the reported employees in part I, line 5, only 51 are employees of consumer directed choices. The remaining individuals reported are those for whom cdchoices provides payroll services on behalf of consumers/designated representatives.

The organization over time
Each line starts at 100 in 2017, so what you read is the shape rather than the size: 150 means half as much again as 2017, 50 means half. The number beside each label in the key is where it ended. The shaded band is where the middle 50% of its peers' revenue would sit, given their growth.

Total assets and total liabilities at each year end, in dollars. The largest one-year move in total assets was 2023 to 2024, from $26M to $34M (+29%).
How it's funded, over time
Each bar is one year's revenue split into where it came from, and every bar is the same height — these are shares, not amounts, so a year that raised twice as much looks the same size. Hover a bar for the split.
Government-grant reliance: 2020 2% · 2021 0% · 2022 0% · 2025 32%. Grants only. Government contracts and fees sit inside program revenue.
32% of CONSUMER DIRECTED CHOICES INC’s revenue is contributions — more donation-reliant than the typical peer (21% for the typical peer).

CONSUMER DIRECTED CHOICES INC leans on a few funders — its largest provides 78% of grant income and the top three 100%; half comes from just 1 funder.
the vertical line marks half of all grant income — 1 funder to its left
78% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund CONSUMER DIRECTED CHOICES INC.
Largest funder’s share by year: 2017 100% · 2022 100% · 2023 100% — broadly stable.
“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration.
