What Coalfields Health Foundation Inc does
Mission
Develop projects to serve our Appalachian regions health and health related needs. First project: raise 12.5 million in capital, and 4 million in operating and endowment funds to build and operate an affordable assisted living facility (ALF) to provide retirement, housing and health related services. Our Appalachian seniors average household income is 44% lower than Virginia and 35% lower than the US. Successful fundraising will enable us to offer this service for almost 40% less than the average ALF rates in Southwest Virginia..
As stated on its FY2025 Form 990.
Programs
Annual updating of the project business plan, demographics, and feasibility study with the latest data, for example, the latest American community survey from the US Census; updating our projected operational financial statements for the ALF; work and review of site, utility infrastructure plans, revising and expanding the ALF facility design plans; planning meetings help with local officials on utilities; working with Freewill Baptist Family Ministries, the company who will be our ALF facility manager of operations regarding our projected financial statements and developing the final draft of our management agreement; updating program plans for the facility; planning the Helping Hands Endowment programs to provide financial subsidies to our lower income, senior residents; updating the fundraising and ALF marketing materials with the new name of the ALF facility.

The organization over time
Each line starts at 100 in 2023, so what you read is the shape rather than the size: 150 means half as much again as 2023, 50 means half. The number beside each label in the key is where it ended. The shaded band is where the middle 50% of its peers' revenue would sit, given their growth.
Balance sheet
Total assets and total liabilities at each year end, in dollars. The largest one-year move in total assets was 2024 to 2025, from $2.5M to $9.4M (+272%).
How it's funded, over time
Each bar is one year's revenue split into where it came from, and every bar is the same height — these are shares, not amounts, so a year that raised twice as much looks the same size. Hover a bar for the split.
Government-grant reliance: 2023 0% · 2024 28% · 2025 0%. Grants only. Government contracts and fees sit inside program revenue.
98% of COALFIELDS HEALTH FOUNDATION INC’s revenue is contributions — more reliant on donations than three-quarters of its peers (39% for the typical peer).

How concentrated its funding is
COALFIELDS HEALTH FOUNDATION INC leans on a few funders — its largest provides 83% of grant income and the top three 100%; half comes from just 1 funder.
the vertical line marks half of all grant income — 1 funder to its left
83% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund COALFIELDS HEALTH FOUNDATION INC.
“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
