Community engagement team members and providers from the medical group volunteer their time at community events to offer preventive care screenings and education. Educational opportunities include engagement during heritage months and health awareness weeks, outreach to underserved communities throughout the year to share health information and promote the need for screenings. The obgyn practice providers partner with local free clinics to host educational baby showers at prince william medical center for mothers-to-be, educating on prenatal care, and providing information about post-partum support groups. The primary care practices participate in an accountable care organization (aco) under the centers for medicare and medicaid services, which holds the entity accountable for the total cost of care and patient health outcomes of their medicare patient panels. Additionally, the pediatric practices participate in fortify children's network, a state-wide clinically integrated network of providers that use data and analytics to improve care coordination and patient outcomes. Uva community health is proud of these population health activities and our providers' success in moving to alternative payment models that provide greater value for the services provided and outcomes…
