01What it does

What Liv Well Cares Inc does

Mission

LivWell CARES (Compassionate Advocacy & Resources for Every Senior) provides free, personalized placement, information, and referral services to low-income seniors and those seeking long-term nursing care, by working together to connect them with senior living solutions and resource referrals that meet their specific needs.

As stated on its FY2025 Form 990-EZ.

Program

  1. LivWell CARES touched the lives of 700 low-income seniors in 2025. In addition to helping seniors find affordable independent senior apartments, residential assisted living, memory care or long-term nursing care, we took on the challenge of coordinating the Low Income Senior Hot Holiday Meal Delivery program for Scott County during the 2025 holiday season. LivWell volunteers personally delivered a total of over 1310 hot meals to the doorsteps of low-income seniors on Thanksgiving and Christmas Days 2025, and New Years Day 2026. Other services included but werent limited to, referrals for transportation assistance, VA benefits, in-home care agencies, Medicaid application assistance, and caregiver support. We collaborate with other local agencies to get seniors the help they need.

03Who funds it

Who funds it, year by year

2020 → 2023: the base broadened from 1 funder to 2 funders, grant income fell $10k → $3k.

1 of 7 of your funders are donor-advised or pass-through sponsors (tagged DAF) — 37% of grant dollars received. That money is really individual donors directing a sponsor; the institutions to cultivate are the non-DAF funders.

From the IRS filings of LIV WELL CARES INC’s funders (the co-funder graph). Association, not causation. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

How concentrated its funding is

LIV WELL CARES INC leans on a few funders — its largest provides 37% of grant income and the top three 70%; half comes from just 2 funders.

the vertical line marks half of all grant income — 2 funders to its left

49% of the income these shares are computed over arrives through pass-through sponsors or from payers whose filings do not say what kind of payment it is. Concentration is still measured over all of it, because the money is real; what it does not support is a claim about how many institutions have chosen to fund LIV WELL CARES INC.

Largest funder’s share by year: 2020 100% · 2021 91% · 2022 42% · 2023 100% — broadly stable.

“Effective funders” = inverse Herfindahl index (1 / Σ shareᵢ²) — the number of equal-sized funders that would give the same concentration. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.

Iowa out of state home

Funder states come from each funder’s own filing. $35k arriving through sponsors registered in 1 state is excluded from the map and the split above: a sponsor holds money on a donor’s behalf, so its registered address would place those dollars somewhere no donor need ever have been. Trends are read to FY2023, the last fiscal year that has finished arriving. FY2024–FY2026 are still being filed and are shown unshaded; they do not move any figure on this page.
